You may request a copy one of three ways:<\/p>\n

  2. Using the on-line request below; or<\/li>\n
  3. Faxing your request to (859) 292-6611.<\/li>\n<\/ol>\n

      2. Fill out the information requested.<\/li>\n
      3. Click the “SEND”\u00a0button at the bottom of the form.<\/li>\n
      4. Copies are $0.25 per page.\u00a0If you would like for your copies to be certified, please indicate so in your request. There is an additional fee of $5.00 per certification.<\/li>\n
      5. If you would like it faxed for e-mailed there is a charge of $2 for the first page and $1 for each page thereafter.<\/li>\n
      6. To have your copies mailed to you, there will be an additional postage fee, depending on how many copies are being mailed.<\/li>\n
      7. Payment for the copies must be made prior to the copies being made<\/strong>.\u00a0Requests are processed in the order they are received.<\/li>\n
