Access to Justice Commission has created a guided interview form that you can print out <a href=\"https:\/\/a2j.org\/guide\/consent\/bb8a72c4-ea99-460d-83be-90779d30a84a\">here<\/a> or you can use the forms below.<\/li>\n<\/ul>\n<p><b>Filing Fees (unless accompanied by a Motion for Leave to Proceed In Forma Pauperis):\u00a0$118.50 plus <a href=\"https:\/\/kentoncourtclerk.org\/?p=1336\">service fees<\/a><\/b><\/p>\n<p>Forms:<\/p>\n<p><b><a href=\"https:\/\/kycourts.gov\/Legal-Forms\/Legal%20Forms\/295.pdf\" target=\"_blank\" rel=\"noopener\">Petition for Name Change \u2013 Form AOC-295<\/a><\/b><\/p>\n<p><b><a href=\"https:\/\/kycourts.gov\/Legal-Forms\/Legal%20Forms\/40.pdf\" target=\"_blank\" rel=\"noopener\">Waiver of Recording \u2013 Form AOC-040<\/a><\/b><\/p>\n<p>All cases also require a Civil Case Cover Sheet which can be <a href=\"https:\/\/kycourts.gov\/Legal-Forms\/Legal%20Forms\/104.pdf\">found here.<\/a><\/p>\n<p><b>The order for name change is not available online but will be provided at time of filing.<\/b><\/p>\n<ul>\n<li>Once a case is filed, and processed by a deputy clerk, it is then forwarded to the judge\u2019s office for ruling, if the petition is for the name change of an adult.<\/li>\n<\/ul>\n<p><b>Special filing requirement for name change of a minor<\/b><\/p>\n<ul>\n<li>If the petition is for the name change of a minor and both biological parents have not signed the petition, a mandatory hearing date is scheduled.\u00a0 The petitioning parent must serve the other parent by certified mail or personal service of the attempt to change the child\u2019s name and also provide them with the assigned court date, location and time.<\/li>\n<\/ul>\n<p>Name Change Hearings are held on Monday afternoons. \u00a0Please refer to the paperwork provided during filing for your scheduled date, time and location.<\/p>\n<p>A name change petition for a child may be filed in Family Court when having a case before it involving the family. When filed in an active circuit or juvenile case there is no filing fee for the name change petition. KRS 401.020.\u00a0 If the case is not active, a re-opening fee may be necessary.<\/p>\n<p><b>If the petition is granted<\/b>, the signed order will then be certified and mailed or hand-delivered \u00a0to the petitioner and\/or attorney.<\/p>\n<p><span style=\"color: #ff0000;\">***Reminder \u2013 Our office is prohibited from providing legal advice.
