There must be\u00a0a serious physical danger to one&#8217;s health\u00a0or damage to or misuse of property if immediate action is not taken.<\/p>\n<ul>\n<li>The filing fee for a petition for emergency guardianship is $75.50.<\/li>\n<\/ul>\n<ul>\n<li>This process begins at the <strong>Kenton County Attorney&#8217;s Office<\/strong> located at 1840 Simon Kenton Way, Covington, Kentucky 41011.\u00a0 Their phone number is 859-491-0600.<\/li>\n<\/ul>\n<ul>\n<li>A hearing will be scheduled within 7\u00a0days.<\/li>\n<\/ul>\n<p>Following are the Emergency Guardianship Forms:<\/p>\n<p><a href=\"https:\/\/kycourts.gov\/Legal-Forms\/Legal%20Forms\/747.pdf\" target=\"_blank\" rel=\"noopener\">AOC-747 Petition \/Application for Emergency Appointment of Fiduciary<\/a><br \/>\n<a href=\"https:\/\/kycourts.gov\/Legal-Forms\/Legal%20Forms\/275.17.pdf\" target=\"_blank\" rel=\"noopener\">AOC-034 Personal Identifier Data Sheet<\/a><\/p>\n<p>Information requested\u00a0to complete emergency guardianship\u00a0petition:<\/p>\n<ul>\n<li>Respondents (person in need of\u00a0guardianship) name and complete address, social security number, date of birth, and a statement from their\u00a0licensed physician.<\/li>\n<\/ul>\n<p>Confidentiality &#8211;\u00a0All court records in disability proceedings are confidential except for the\u00a0order of appointment.<\/p>\n<ul>\n<li>Any person seeking to obtain confidential information contained in the court files or records may file a written motion stating why the information is needed. \u00a0The Circuit Clerk will maintain a log of individuals and entities granted access to the files or records by court order.<\/li>\n<\/ul>\n<p><strong>Additional forms in emergency\u00a0guardianship cases<br \/>\n<\/strong><a href=\"https:\/\/kycourts.gov\/Legal-Forms\/Legal%20Forms\/855.pdf\" target=\"_blank\" rel=\"noopener\">60 Day Inventory or Supplemental Inventory<\/a><strong><br \/>\n<\/strong><\/p>\n<p>All cases also require a Civil Case Cover Sheet which can be <a href=\"https:\/\/kycourts.gov\/Legal-Forms\/Legal%20Forms\/104.pdf\">found here.<\/a><\/p>\n<p><span style=\"color: #ff0000;\">***Reminder \u2013 Our office is prohibited from providing legal advice.
