  3. Faxing your request to (859) 292-6611. PRINT FORM
  4. Please allow up to two (2) weeks for duplication.

AV Duplication Request

    3. Click the “SEND” button at the bottom of the form.
    4. To have your copies mailed to you, there will be an additional postage fee, depending on how many copies are being mailed.
    5. Payment for the copies must be made prior to the copies being made. Requests are processed in the order they are received.
    6. YOU WILL BE CONTACTED TO ARRANGE PAYMENT.
